맥스 페인은 수년 전에 자신이 사랑하던 사람들을 잃는 비극을 겪었지만 그 상처가 치유되는 것을 거부하고 있습니다.
경찰에서 물러나 심신이 망가진 채 진통제에 의지하며 거의 폐인처럼 살아가던 맥스는 끔찍한 과거로부터 벗어나기 위해 브라질 상파울루에서 부동산 거물 갑부 로드리고 브랑코의 집안을 경호하는 일거리를 맡게 됩니다. 하지만 고약한 운명의 장난처럼 사건이 발생하고 낯선 도시의 길거리에 홀로 남게 된 맥스 페인은 절박하게 진실을 쫓으며 살아남기 위해 분투합니다.

초반에는 스타일리시한 슬로우 모션을 곁들인 시원스런 액션게임 이었는데 가면 갈 수록 적들 피통은 돼지같이 커지고 끝도 없이 나와 엄폐만 하며 짤짤이로 싸워야함. 그 와중에 엄폐할 곳이 없는건 덤. 후반에 게임이 지나치게 루즈해짐. 만약 한다면 반드시 쉬움 난이도로 할 것.
